The 2025 Community Health Needs Assessment (CHNA) was distributed to members of the Community Benefit Initiative Committee at Community Hospital San Bernardino and to hospital board members and partner organizations. The assessment was made available in both electronic and printed formats, ensuring broad accessibility and convenience for all stakeholders. The hospital also shared the CHNA with all community non-profit organizations that considered applying or did apply for a Community Health Improvement Grant. CHNA significant needs are integral to these restricted contributions for services that address the health and well-being of vulnerable and underserved populations.
