License Facility Information
Facility Profiles – Interactive map to find a summary profile of facility information, including license, service level, revenue, payer mix, length of stay, and building safety information. Use the map or search functions to find hospital, long-term care, clinical, home health, and hospice facilities.
Licensed Facility Information System (LFIS) – View facility license information of California hospitals, long-term care facilities, primary care and specialty clinics, home health agencies and hospices.
Licensed Healthcare Facility Listing – A list of California healthcare facilities licensed by California Department of Public Health, Licensing and Certification.
Licensed Facility Crosswalk – This dataset provides a simple crosswalk using Department of Health Care Access and Information (HCAI) assigned licensed facility identification numbers linked with matched California Department of Public Health (CDPH), Licensing and Certification facility lists based on license number. This is not a comprehensive matched list, facility identification numbers that did not match are also included from both the HCAI and CDPH lists. Facility Status or Facility Level designations may explain some HCAI non-matches, for additional information contact HCAI directly. Please contact CDPH directly for more information regarding un-matched facility identifiers that do not have corresponding HCAI identifiers.

Hospital Building Data – Provides the basic information for general acute care hospital buildings such as height, number of stories, the building code used to design the building and year it was completed. The data is sorted by counties and cities. Structural Performance Categories (SPC ratings) are also provided. SPC ratings range from 1 to 5 with SPC 1 assigned to buildings that may be at risk of collapse during a strong earthquake and SPC 5 assigned to buildings reasonably capable of providing services to the public following a strong earthquake. Where SPC ratings have not been confirmed by the Department of Health Care Access and Information (HCAI) yet, the rating index is followed by ‘s’. A URL for the building webpage in HCAI/FDD eServices Portal is also provided to view projects related to any building.
Seismic Compliance Data by County for General Acute Care Hospitals – Data for seismic compliance in general acute care hospitals is grouped by counties. All general acute care hospital buildings are assigned a structural performance category (SPC). SPC ratings range from 1 to 5 with SPC 1 assigned to buildings that may be at risk of collapse during a strong earthquake and SPC 5 assigned to buildings reasonably capable of providing services to the public following a strong earthquake. State law requires all SPC 1 buildings to be removed from providing general acute care services by 2020 and all SPC 2 buildings to be removed from providing general acute care services by 2030. Therefore, a hospital facility that may be comprised of a number of hospital buildings meets the 2020 requirements if there are no SPC 1 buildings on campus. A hospital facility meets the 2030 requirements if all the buildings on campus are either SPC 3, 4 or 5. Data is provided for both hospital facilities and hospital buildings. Data is updated approximately every two weeks.
Seismic Deadline Extension Granter for California Hospitals – This dataset lists the General Acute Care (GAC) Hospitals that have been granted extensions for seismic compliance; facilities that currently have extension requests under review are not included in this table. Extensions are sorted by County, by hospital building.
Seismic Ratings and Collapse Probabilities of California Hospitals – This dataset lists the Structural Performance Category or “SPC rating” and Nonstructural Performance Category or “NPC rating” of all General Acute Care Hospitals in California, including a building’s probability of collapse in a significant earthquake is assessed using Multi-Hazard Loss Estimation Technology (HAZUS).
Total Construction Costs of Healthcare Projects – Total dollar value and number of projects either in review, pending construction, in construction, or in closure aggregated into California counties, once every two weeks since September 2013. A construction project moves through the Department of Health Care Access and Information (HCAI) in four stages – In Review; Pending Construction Start; Under Construction; and In Closure. A project can only be in one of these four stages at any time. Additional data when available will be added to this dataset approximately once every two weeks.
