How the hospital made the Community Health Needs Assessment (CHNA) available to the public:
FHMC 2025 CHNA report was sent electronically to community partners, community benefit committee members, and hospital board members. The hospital also shared the CHNA with all community non-profit organizations that considered applying or did apply for a Community Health Improvement Grant. CHNA significant needs are integral to these restricted contributions for services that address the health and well-being of vulnerable and underserved populations.
