The Inland Empire Community Health Needs Assessment (CHNA) shines a light on both the challenges and opportunities facing the people of Riverside and San Bernardino counties.This collaborative effort was led by Communities Lifting Communities (CLC), an affiliate of the Hospital Association of Southern California (HASC), in partnership with five local hospitals: Chino Valley Medical Center, Montclair Hospital Medical Center, Redlands Community Hospital, San Antonio Regional Hospital, and San Gorgonio Memorial Hospital. What makes this report unique is that it combines data on disease burden, vital conditions, and hospital utilization with the voices of more than 11,000 residents, health care professionals, and community leaders. Through community input via wellbeing surveys and hospital data, vulnerable populations were identified as individuals experiencing homelessness, people living with economic hardship, those facing insurange gaps and access barriers, youth, older adults, communitities of color and women.
