Saint Agnes Medical Center (SAMC) is dedicated to improving healthcare access for all individuals, regardless of race, culture, or economic background. We prioritize vulnerable populations to ensure our programs effectively serve those most in need. SAMC defines vulnerable populations as people living in poverty. Our focus areas include communities where at least 40% of residents are Black, Hispanic, or Latinx, or where the median income is at or below 200% of the federal poverty level. To ensure vulnerable populations' voices are heard in the latest Community Health Needs Assessment (CHNA), SAMC collaborated with public health and community stakeholders. This involved conducting surveys, key informant interviews, and focus groups to identify community needs. We partnered with community-based organizations such as Cultiva la Salud, the Fresno Center, and United Way of Fresno and Madera Counties to represent underserved groups, including those from low-income and minority backgrounds, along with a subcontract with Live Well Madera County for primary data collection. The Central Valley Health Policy Institute (CVHPI) analyzed both qualitative and quantitative data, helping identify major health needs in Fresno and Madera Counties. This analysis resulted in a list of significant health issues for these areas. SAMC recognized the various health concerns identified during the CHNA and decided to focus on addressing the most urgent and neglected needs that it can effectively influence, including 1) food insecurity, 2) safety and neighborhood crime, and 3) insufficient providers, treatment locations, and long wait times.
