The year the hospital last conducted a Community Health Needs Assessment (CHNA):
2025
What community groups attended or engaged with the most recent CHNA process? Identify the vulnerable populations represented by these:
Community stakeholder interviews were conducted in collaboration with two hospitals in the region. Thirty-three (33) stakeholder interviews were completed from October to November 2024. Interview participants included a broad range of stakeholders concerned with health and wellbeing in the South Bay, who spoke to issues and needs in the community. Stakeholders included individuals who are leaders and representatives of medically underserved, low-income, and minority populations, or local health or other departments or agencies that have "current data or other information relevant to the health needs of the community served by the hospital facility.” The identified stakeholders included, but were not limited to, organizations focused on low-income, older adults, people with disabilities and people experiencing homelessness, as well as organizations serving primarily BIPOC (Black, Indigenous and People of Color) populations.
Does the Hospital make the CHNA report widely available to the public?
Yes
How the hospital made the Community Health Needs Assessment (CHNA) available to the public:
The hospital made the 2025 Community Health Needs Assessment (CHNA) publicly accessible by posting the full report on our website, where it can be viewed or downloaded at any time. The prior 2022 CHNA is also available online in the same section. Both reports are located in an open, easy-to-find section dedicated to Community Benefit, ensuring broad public access for patients, community members, partner organizations, and other stakeholders. Copies or assistance accessing the reports are also available upon request through our online Community Benefit feedback form or via the hospital’s Contact Us webpage.
