2. Equity Plan

Our critical access hospital is actively addressing 3 of the 10 identified disparities within our community, we are committed to continuous improvement in health care equity. Currently Ridgecrest Regional Hospital has a workgroup that specifically addresses all-cause 30-day readmissions. Our analysis of the existing data shows that our female Medicaid population aged 50 to 64 has the greatest chance pf readmissions verses our male population with the same demographics. We are instituting a trial discharge clinic to ensure that upon discharge our high-risk patients will be seen by a provider in the interim to seeing their established provider. The provider will address education needs to include medications. disease process, and specific needs to the individual patients in attempt to reduce readmissions. Our goal is to have the trial clinic opened by December 2025 with a noted 3% reduction in readmissions by June 1, 2025.
