Disparities for each hospital equity measure are identified by comparing the rate ratios by stratification groups. Rate ratios are calculated differently for measures with preferred low rates and those with preferred high rates. Rate ratios are calculated after applying the California Health and Human Services Agency's "Data De-Identification Guidelines (DDG)," dated September 23, 2016.

2. Equity Plan

The development of robust action plans to address the disparities identified in this submission requires further in-depth analysis and contextual understanding. The current data reveals significant disparities across multiple domains but also presents challenges related to data definitions, small sample sizes, and the clinical complexity of the high-risk population served by the San Francisco Health Network (SFHN). For example, pneumonia-related outcomes show disproportionately high rates among Asian and Asian Pacific Islander language speakers, while readmission rates are notably elevated among Medicare, Medicaid, and LGBTQ+ populations. These disparities are further compounded by social drivers of health and systemic barriers to care. Recognizing these complexities, SFHN remains firmly committed to advancing health equity. As part of its 2026 Strategic Plan, our organizations will prioritize targeted, data-informed initiatives to reduce disparities. This includes refining data collection and stratification methods, engaging impacted communities in solution design, and implementing measurable interventions to improve outcomes for the diverse populations we serve.
