California is taking an important step toward transforming rural healthcare by distributing $166.72 million in California Rural Health Transformation Program funding for 108 organizations across 45 counties.

These investments will help rural communities expand primary, maternity, specialty, and whole person care while strengthening the providers and facilities on which rural Californians depend.

Transformative Care Model

Approximately $87.51 million supporting 34 recommended grants, along with milestone-based hospital investments and clinical training partnerships, to expand primary, maternity, specialty, and whole person care closer to home.

Workforce Development

Approximately $67.56 million supporting 65 recommended grants to strengthen recruitment and retention, develop rural career pathways and expand clinical training opportunities.

Technology and Tools

Approximately $11.65 million supporting 9 recommended grants to help rural, remote, and Tribal providers modernize electronic health records, strengthen cybersecurity, and improve patient data exchange.

Tribal Set-Aside

CalRHT also includes dedicated funding to support California’s Tribal communities and rural Tribal healthcare delivery system.

Five percent of California’s first-year CalRHT funding, totaling $11.65 million included within the initiative funding described above, is reserved for California’s federally recognized Tribes and the rural Tribal healthcare delivery system. The allocation was developed in consultation with Tribal leaders.

This support aims to expand access to care, improve the quality and coordination of services and help Tribal communities build stronger and more sustainable local health systems.

Contract and Program Support

In addition to these recommended grant investments, Budget Period 1 funding will support the infrastructure and assistance needed to administer CalRHT and help participating organizations implement their work.

CalRHT includes $56.51 million in contract support, including funding the Rural Technical Assistance Center (RTAC), and $10.41 million in HCAI program support.

Together, these recommended investments will help expand access to care, strengthen the rural healthcare workforce, and modernize healthcare technology in rural communities across California.

The organizations recommended for Budget Period 1 funding are listed below alphabetically by initiative and grant. The list reflects awards announced to date and does not yet represent all Budget Period 1 investments included in the initiative totals above. Additional recommendations will be added as they are ready for publication.

Funding amounts shown reflect the amounts offered to potential subrecipients and may differ from the final amounts disbursed. This information will be updated weekly as subrecipient agreements are executed.

All Budget Period 1 funding recommendations are subject to completion of the grant agreement and contracting process. Inclusion on this list does not constitute a final grant award.

The Accelerator Partner Program is proposed as support for hospitals or other organizations in rural regions that demonstrate strong readiness and commitment to implement the CalRHT Transformative Care Model (TCM) through regional care collaboratives and hub-and-spoke relationships which will seek to strengthen primary and maternity care in rural parts of the state.
