The 2025 Community Health Needs Assessment reflects the insights of 1,304 community members and leaders across San Bernardino and Riverside Counties through a combination of 1,138 community surveys, 21 community conversations facilitated in English and Spanish, and 16 key informant interviews. Surveys were primarily collected through community-based partner outreach, with approximately 20 percent collected in LLUH emergency department waiting rooms. We partnered with more than 30 trusted organizations spanning public health, education, community-based nonprofits, faith-based groups, healthcare access partners, and organizations that work daily with residents facing economic, social, and health-related barriers. These partnerships ensure participation from vulnerable communities most impacted by health inequities, including residents with limited access to affordable healthcare, families experiencing financial strain, communities of color, immigrants and mixed-status families, individuals with limited English proficiency, people living with disabilities, unhoused individuals, and residents of neighborhoods facing environmental and economic challenges. By relying on longstanding relationships, culturally responsive outreach, and accessible engagement methods, the CHNA process elevated voices that are often underrepresented in traditional planning efforts and ensured that community priorities reflected lived experience across the region.

Loma Linda University Health made the 2025 Community Health Needs Assessment publicly available by posting the full report as a PDF on its website, where it remains accessible to community members and stakeholders. A printed copy of the report is also available upon request. The CHNA was shared directly with participating community partner organizations, members of the Community Benefit Administrative Council, and the LLUH Board of Trustees, and was presented across multiple internal and external forums, including with hospital leadership, staff, emergency department teams, and community-based organizations. The CHNA was also shared with nonprofit organizations applying for Community Health Investment Awards, where alignment with identified CHNA priorities is required.
