Each report includes hospital- or hospital system-level measures of access, quality, and outcomes stratified by demographic and socioeconomic factors, along with a plan to address top disparities identified in the data.

Hospitals that did not submit a report are listed separately on the CalHHS Open Data Portal (ODP), “List of hospitals/hospital systems that failed to submit an equity report to the Department.”
