AURORA VISTA DEL MAR HOSPITAL
1. Top 10 Disparities
The following table lists the ten largest health equity disparities identified for this reporting period.
Disparities for each hospital equity measure are identified by comparing the rate ratios by stratification groups. Rate ratios are calculated differently for measures with preferred low rates and those with preferred high rates. Rate ratios are calculated after applying the California Health and Human Services Agency's "Data De-Identification Guidelines (DDG)," dated September 23, 2016.
The table below highlights the ten widest health equity disparities identified by hospitals and hospital systems during this reporting period. Measure names have been shortened for display purposes. 2. Equity Plan
Aurora Vista del Mar could not find the differences in care among important patient groups for 2024. However, we have created a Health Equity Plan to make sure we address these differences in the future for our patients. The plan focuses on improving key areas, especially understanding how these differences affect getting care and patient results. We have asked our IT department to find ways to bring together the data we already collect for different groups so we can look at results by Race and/or Ethnicity, Age, Sex Assigned at Birth, Payer Type, Preferred Language, Disability Status, Sexual Orientation, and Gender Identity. This will help us look at the data for each group and compare it to national data. We are also creating training for our Intake, Social Services, and Clinical teams who collect this data to make sure it is complete, correct, and gathered in ways that respect different cultures. We are also making resources for each group to help with education about care and actions that are specific to each culture and that address possible and real differences in care. This will: (a) help our Intake team show respect for different cultures when assessing patients and keep training them in the best ways to work with people from different backgrounds and the different ways symptoms can show up in each group;
(b) help our Clinical Services teams learn about the best ways to assess and treat each group so we can offer the most up-to-date, patient-focused care that understands how differences in care can affect our patients’ mental health, substance use, and overall well-being;
(c) help our Social Services and Case Management teams give patients the right resources for aftercare and planning for when they leave, based on their unique cultural needs; (d) guide our Business Development and Community Outreach teams to find new partners and resources in the community that meet the needs of our different groups, and to keep getting feedback from these communities about how well our programs and care are working; and help our Quality Improvement department better track how we are doing with these groups using important measures.
