2. Equity Plan

Community Memorial Hospital - Ventura recognizes that nine out of ten disparities reflected in our data are related to readmissions within 30 days and are closely tied to the demographics of our service area, including race, ethnicity, age, and payor type. While we cannot control the underlying community demographics, we can address the way these factors influence health outcomes by focusing on equitable access and treatment by implementing focused strategies that lessen their impact and advance equity in care delivery. The overall goal of reducing readmissions by 10% has been identified as one of main strategies for better patient outcomes. Patients are assessed for risk of readmissions and followed by case managers when moderate to high risk is identified. The Community Memorial Case Management department is collaborating with a transitional care management program for Medicare patients discharged to home, which involves initial contact within 48 hours, and 30 days follow up for medication reconciliation and support. Patients also receive post-discharge care through our Transition Care Management program at our clinics.
Community Memorial Hospital - Ventura received Geriatric Emergency Department Accreditation by ACEP in 2025. This accreditation is awarded to organizations who demonstrate their commitment to adopting best practices for older adults in the emergency department and are dedicated to providing the best care possible for the community's older adults.
The last disparity identified was related to breast feeding. We pride ourselves on having received the Excellence in Lactation Care Award, given by IBLCE and ILCA, as well as the Maternity Honor Role from the California Health and Human Services Agency. CMH- Ventura Community Memorial Hospital - Ventura has also been a designated Baby-Friendly Birth Facility for many years. This designation recognizes that a maternity facility has successfully implemented the Ten Steps to Successful Breastfeeding as stipulated by the WHO and Baby-Friendly USA. We will continue to provide education and support to all mothers who deliver their babies at our facility, and will work with our physicians on providing breast feeding education and other perinatal care to expecting mothers prior to their delivery date.
