Disparities for each hospital equity measure are identified by comparing the rate ratios by stratification groups. Rate ratios are calculated differently for measures with preferred low rates and those with preferred high rates. Rate ratios are calculated after applying the California Health and Human Services Agency's "Data De-Identification Guidelines (DDG)," dated September 23, 2016.

2. Equity Plan
Maternal Health:
Current focus on maternal health in Black women and will now include Asian and Hispanic patients as well. CHS partners with Black Wellness and Prosperity Center for prenatal and postnatal. CHS supports Duala training.
CHS system focus on decreasing cesarean sections which includes our patients who utilize private pay.
Mothers Resource Center offers breastfeeding support.
Readmissions:
Several physician-led projects in 2025 that focused on decreasing readmissions for patients with pneumonia and COPD.
CHS partners with Fresno Mission for the Fresno Medical Respite Center which allows patients who experience homelessness to have intermediate care after hospitalization.
CHS has a new post-acute care Skilled Nursing and Rehabilitation Center that has just opened.
Multidisciplinary care groups for discharge planning that includes ensuing patients have appropriate follow-up.
CHS participates in a Meds to Beds program which ensures patients have the right medications at discharge.
Medical equipment for home use.
CHS is beginning several quality-related projects which includes reducing our readmission rates for all patient groups.
Utilize a Readmission Risk Score in EMR.
Pneumonia Mortality:
Physician-led project to decrease pneumonia readmissions which also impacts pneumonia mortality.
CHS system focus on readmissions in our population aged 65 and older which includes Medicare population by implementing multidisciplinary rounds and connecting patients with appropriate pulmonary follow-up post-discharge.
