2. Equity Plan

East Los Angeles Doctors Hospital is committed to promoting health equity and improving patient outcomes by addressing the disparities identified in our AB-1204 assessment. To achieve this, the hospital is enhancing communication with patients, improving the clarity and consistency of education provided at discharge, and expanding support for individuals with chronic conditions or social needs that may affect their care. We are adopting evidence-based practices—such as teach-back, culturally responsive communication, and strengthened care coordination—to ensure every patient receives the information and support needed to safely manage their health. By tracking progress, engaging staff in ongoing training, and partnering with patients and families, the hospital will reduce gaps in patient experience and readmissions and advance equitable, high-quality care for our diverse community.
