Rate ratios are calculated differently for measures with preferred low rates and those with preferred high rates. 2. Equity Plan
Measures and Targets:
Reduce 30day readmission disparities for Black or African American patients from 25.9% toward parity with White patients (14.2%) by Q4 FY2026.
Decrease older adult readmission rates from 18.6% to less than or equal to 15% through geriatric discharge planning.
Narrow male or female readmission gap by greater than or equal to 20% with targeted education and follow-up.
Overall Medicaid readmission rates reduced from 15.6% to less than or equal to 13.5% by Q4 FY2026.
