2. Equity Plan

Rancho Los Amigos National Rehabilitation Center (Rancho) is uniquely positioned to address health disparities because our primary focus is on patients with disabilities and providing acute rehabilitation services within a safety net system. As such, we have robust systems including 1) a robust integrated patient and family advisory committee that is actively involved in quality improvement projects within the hospital, 2) an interdisciplinary patient experience council to review and address areas of concern, 3) an interdisciplinary patient safety council trying to assure a culture of safety is up front and center in the care we provide. For our patients with higher readmissions based on age of 65 and older, we will be carefully assessing this population to determine possible steps to assure close follow up or needs assessment to determine unmet needs of this population. We will monitor readmission rates in this age cohort from our Vizient and HQI analytics on a quarterly basis.
