2. Equity Plan

As one of four hospitals operating under the consolidated license for Santa Clara Valley Medical Center (SCVMC) and part of the larger Santa Clara Valley Healthcare (SCVH) system, St Louise Regional Hospital (SLRH) maintains clear policies that prohibit discrimination in both patient care and staff conduct. These expectations are reinforced through staff orientation and ongoing compliance training. Healthcare incident reports are reviewed at the executive level and monitored in partnership with the Office for Health Equity & Improvement to ensure accountability and to address systemic trends.

Readmissions are an area of focus for SLRH, with efforts being made to improve support to reduce unnecessary readmissions. For example, data suggests that there is opportunity to reduce readmission in older age groups and SLRH is implementing initiatives that align with age-friendly programs to improve care coordination and communication with older patients who require hospitalization.
