2. Equity Plan

Valley Presbyterian Hospital (VPH) is committed to improve patient health outcomes and reduce health-care disparities through data-driven health equity interventions and quality services. Upon reviewing the top 10 disparities they can be sub-categorized into two main focuses. Disparity 1 & 2 is quality indicator pneumonia mortality rate with white race and all males being the top disparity. The best performing reference groups are Latinos and females by contrast. Disparity 3 - 10 are all focused on all cause 30-day readmission rates with stratification identifying ages 35 and above, along with white and black - African Americans. VPH will continue to develop a plan by assessing the needs of the populations identified in the top 10 disparity list and stratify the data further to identify specific areas to address. In the next 3-6 months VPH will further stratify related to mortality and readmissions listed above to identify 2-3 modifiable attributing factors and create an action plan to deliver changes.
