Our CHNA process engaged a wide range of community groups, including public health agencies, community-based organizations, nonprofit partners, and healthcare providers that serve diverse and high-need populations. These stakeholders represented vulnerable populations at increased medical and financial risk, including individuals who are uninsured or underinsured, as well as those eligible for Medi-Cal or Medicare. In addition, the process incorporated perspectives from racial and ethnic communities experiencing disproportionate health outcomes, along with other socially disadvantaged populations such as low-income individuals, seniors, and individuals facing barriers to accessing care. This collaborative approach helped ensure that the needs, challenges, and priorities of underserved communities were accurately identified and reflected in the CHNA.
