Community Groups Engaged in the 2025 Community Health Needs Assessment NorthBay Health's 2025 Community Health Needs Assessment (CHNA) engaged 16 community organizations and public agencies through in-depth key informant interviews conducted between February and November 2024. Participants were selected for their leadership roles and expertise in serving vulnerable and underserved populations throughout Solano County and the broader NorthBay service area. Interviews were conducted with organizations from public health, behavioral health, primary care, education, aging services, workforce development, housing and homelessness, food security, community safety, youth development, and community-based social services. Participating organizations included: Aldea Children & Family Services Children's Network of Solano County CommuniCare + OLE Health Fighting Back Partnership Napa/Solano Community Health Initiatives Napa/Solano Area Agency on Aging / Meals on Wheels Solano Rio Vista CARE S.O.A.R. (Students Overcoming Adversity & Recidivism) Solano County Behavioral Health Solano County HEALs (Health Equity for African American/Black Lives) Solano County Office of Education Solano County Public Health Sustainable Solano Workforce Development Board of Solano County Yolo County Health & Human Services/Public Health Eric Reyes Foundation Vulnerable Populations Represented Collectively, these organizations represented a broad cross-section of vulnerable and underserved populations, including: Low-income individuals and families Medically underserved populations Racial and ethnic minority communities experiencing health disparities, including African American/Black, Hispanic/Latino, Asian, and other diverse populations Children, adolescents, and transition-age youth Older adults and seniors Individuals experiencing homelessness or housing instability People with behavioral health and substance use disorders LGBTQ+ individuals Individuals with limited English proficiency Immigrants, refugees, and undocumented residents Seasonal and agricultural workers Justice-involved and at-risk youth Individuals living in rural and geographically isolated communities Residents facing environmental health challenges, including inadequate access to safe drinking water or clean air. Assessment The breadth of organizations engaged ensured that the CHNA incorporated perspectives from providers and community leaders serving populations with the greatest health disparities and barriers to care. This multidisciplinary engagement informed the identification and prioritization of NorthBay Health's six significant community health needs: access to care, mental and behavioral health, housing, income and employment, community safety, and education.

The 2025 NorthBay Health Community Health Needs Assessment (CHNA) was approved by the NorthBay Health Board of Directors and made publicly available through the NorthBay Health website. The report is available for community members, partner organizations, and other stakeholders to review and informs the development of NorthBay Health's Community Benefit Plan and implementation strategies. The CHNA findings are also shared with community partners and local organizations that participated in the assessment process to support collaborative efforts to address identified health priorities.
