Interviews were conducted with community stakeholders who represent medically underserved, low-income, and/or minority populations to obtain input on health needs, barriers to care, and resources available to address identified significant health needs. The community agencies and organizations included, North Valley Caring Services, El Centro de Amistad, Valley Community Healthcare, Valley Presbyterian Hospital, Eyes of a Mother, Central Lutheran Church, ONEgeneration, San Fernando Valley Community Mental Health Center, MEND: Meet Each Need with Dignity, The Help Group Project Safe, 211 LA, Northeast Valley Health Corporation, Comprehensive Community Healthcare Centers, Vida Mobile, LA County Department of Public Health. Additionally, community residents were engaged in the CHNA process. The vulnerable populations represented by these groups included those exposed to medical or financial risk by virtue of being uninsured, underinsured, or eligible for Medi-Cal and/or Medicare, racial and ethnic groups experiencing poor health outcomes, the unhoused, individuals identifying as LGBTQ, people with disabilities, people with limited English proficiency, individuals and families experiencing food insecurity, and service area communities with a California Healthy Places Index score of 10.4% for clean environment.
