The following table lists the ten largest health equity disparities identified for this reporting period.
Disparities for each hospital equity measure are identified by comparing the rate ratios by stratification groups. Rate ratios are calculated differently for measures with preferred low rates and those with preferred high rates. Rate ratios are calculated after applying the California Health and Human Services Agency's "Data De-Identification Guidelines (DDG)," dated September 23, 2016.

2. Equity Plan
There are a high amount of "not disclosed" data for sexual orientation, disability, and gender identity which skews the ratio and prevents us of accurately identifying disparities. There is also no "other" or "unknown" in the race and/or ethnicity section. Our plan is to ensure staff are trained to ask each patient of the categories with high "not disclosed" data so that we can accurately identify the disparities in our patient population. The disparity we will focus on is the "HCAI All-Cause Unplanned 30-Day Hospital Readmission Rate" for co-occurring disorders for 65 and older patients. The Clinical Services team will provider additional resources specific to those 65 and older with mental health disorders and substance use disorders.
