2. Equity Plan

To address our top ten disparities that were identified, we have implemented plans that emphasize population-level impact, measurable objectives, and clear timeframes. Actions will include the use of evidence-based, targeted interventions tailored to the needs of the most affected groups, along with strong engagement of community partners, local organizations, and stakeholders to ensure approaches are culturally responsive and accessible. Policy and system-level changes will also be prioritized to remove barriers and eliminate practices that unintentionally perpetuate disparities. These priorities have been formally incorporated into the organization's strategic plan, and findings were further informed by the most recent Community Health Needs Assessment (CHNA). To extend reach and impact, partnerships have also been established with community-based organizations (CBOs) to perform targeted outreach to at-risk populations and improve connections to care. Progress will be closely monitored through data collection and analysis, with outcomes. Specific measurable objectives will include improvements in access, reductions in outcome gaps, and increased patient and community satisfaction. Together, these actions create a structured, time-bound approach that aligns strategies with the goal of advancing equity and improving outcomes across all identified disparities.
