2. Equity Plan
Coast Plaza Hospital will address the ten identified disparities through a coordinated, equity-focused improvement plan designed to strengthen communication, enhance patient education, standardize discharge practices, and improve care transitions across demographic groups. Key interventions include expanding culturally responsive communication, reinforcing AIDET and teach-back, implementing bilingual and literacy-sensitive education materials, and increasing caregiver involvement in discharge processes—particularly for older adults and racial/ethnic groups with lower experience scores. To reduce readmissions among Hispanic/Latino, Black or African American, White, and older adult populations, the hospital will enhance risk stratification workflows, expand pharmacist-involved medication reconciliation, encourage timely follow-up appointments, and provide targeted post-discharge care instructions for high-risk groups. Social needs screening, community partnerships, and care navigation support will help address underlying drivers of preventable readmissions. Through clear measurable objectives, routine leadership review, and standardized performance tracking, Coast Plaza Hospital will reduce disparities in patient experience, improve education and understanding of care plans, and strengthen equitable, safe, and coordinated care for all patients.
