2. Equity Plan

Community Hospital of the Monterey Peninsula (CHOMP), part of Montage Health, is focusing on reducing hospital readmissions in 2026. We've found that some groups of patients - such as older adults, people with Medicare or Medicaid, and certain racial and ethnic communities - are more likely to return to the hospital within 30 days of discharge. We want to change that. To help, we've created a Readmission Reduction Workgroup that will use national best practices and improve how we collect and use data about patients' health and social needs. This will help us better understand what drives readmissions and how to prevent them. Our data shows that readmission rates for conditions like heart failure, COPD, pneumonia, and joint replacements are higher than benchmarks, especially for vulnerable populations. To address this, we're using tools that identify patients at higher risk and connecting them with extra support - such as follow-up phone calls, care coordination, and chronic care management. We're also making it easier to get care when you need it. Montage Health is expanding same-day care options through our Call Us First campaign, more urgent care centers, telehealth visits, and additional providers. Finally, we're proud to participate in Monterey County's Community Health Needs Assessment every three years. This helps us and our partners understand local health challenges and plan solutions that make a real difference.
