Disparities for each hospital equity measure are identified by comparing the rate ratios by stratification groups. Rate ratios are calculated differently for measures with preferred low rates and those with preferred high rates. Rate ratios are calculated after applying the California Health and Human Services Agency's "Data De-Identification Guidelines (DDG)," dated September 23, 2016.

2. Equity Plan
EPHC is focusing on improving disparities across multiple domains as part of the organization's strategic initiative. These include:
• Chronic disease management for Medi-Cal recipients
o (e.g., hypertension, diabetes, smoking cessation, cervical cancer screening, and mammography)
• Behavioral health (e.g., depression, substance use)
• Preventive care coverage
• Transportation, food insecurity, and housing through our ECM CalAIM program
• Language/cultural access utilizing interpreter services
• Financial access (medical debt/uninsured status) through our financial eligibility department.
• Modification of our service locations for handicapped patients through the implementation of automatic doors, infection control infrastructure barriers, and exam room renovations.
EPHC tailors the care plans to meet the unique needs, values, and goals of each patient, especially important in rural settings where resources may be limited.
