2. Equity Plan

Our data identified disparities in readmissions, ED utilization, clinical decision making, patient satisfaction, social needs, mortality rates, and preventative screening affecting age groups, race groups, sex differences, SDOH, language, and insurance status. Addressing potential inequities with an emphasis towards patient-centered care is central to our mission of delivering high-quality care to all patients. We have launched targeted interventions, including interpreter services, social services, community partnerships, post-discharge care transition support, patient services in the hospital, care transition clinics for underserved patients, and more. These initiatives are designed to close gaps in access and outcomes. We have begun establishing measurable objectives, such as reducing the disparity gap in satisfaction levels by age, readmissions by diagnosis and race group, access to care by insurance type, and access to services by social determinants of health. Progress is tracked quarterly through our executive committee focused on equity and patient-centered care.
