2. Equity Plan

To address potential inequities, we have in place language interpreter services and a robust post-discharge follow-up program to reduce avoidable readmissions due to patients lacking necessary medications or failure to attend follow-up appointments. Our 2026 strategic planning initiatives include increate mitigation for patients along with outcome measurements based on data collected on persons served including but not limited to gender, age, race, payer source and patient reported determinants of health.
