The following table lists the ten largest health equity disparities identified for this reporting period.
Disparities for each hospital equity measure are identified by comparing the rate ratios by stratification groups. Rate ratios are calculated differently for measures with preferred low rates and those with preferred high rates. Rate ratios are calculated after applying the California Health and Human Services Agency's "Data De-Identification Guidelines (DDG)," dated September 23, 2016.

2. Equity Plan
Maternal health has always been a high priority for Community Health System. Our Mother’s Resource Center, which provides breastfeeding and parent education to new parents, regardless of where they delivered their babies, provided almost 1,000 inpatient consults and over 600 outpatient consults. CHS will continue to use this resource to connect more with our Asian and Hispanic populations.
Another area of focus for CHS is decreasing the rate of cesarean sections, including those mothers with previous cesarean sections. We have partnered with the BLACK Wellness and Prosperity Center for a first-of-its-kind in the state collaboration to train BIPOC doulas within our facilities so that we may provide culturally relevant care to our patients.
Transitions of care programs are the cornerstone for supporting our patients on their recovery journey after discharge, with reductions in readmissions and mortality. In 2025, CHS supported several physician-led projects to decrease readmissions and mortality in patients with pneumonia and COPD. We partner with Fresno Mission for the Fresno Medical Respite Center which allows patients who experience homelessness to have intermediate care after hospitalization. CHS just opened Burnett Extended Care Center that provides skilled nursing care and rehabilitation. Our outpatient pharmacy and case managers work together on the Meds to Beds program, ensuring patients have the medications they need prior to discharge.
