1. Top 10 Disparities

The following table lists the ten largest health equity disparities identified for this reporting period.

Disparities for each hospital equity measure are identified by comparing the rate ratios by stratification groups. Rate ratios are calculated differently for measures with preferred low rates and those with preferred high rates. Rate ratios are calculated after applying the California Health and Human Services Agency's "Data De-Identification Guidelines (DDG)," dated September 23, 2016.

The table below highlights the ten widest health equity disparities identified by hospitals and hospital systems during this reporting period. Measure names have been shortened for display purposes. 2. Equity Plan

Community Memorial Healthcare recognizes that all of the disparities reflected in our data — including race, ethnicity, age, and payor type - are closely tied to the demographics of our service area. While we cannot control the underlying community demographics, we can address the way these factors influence health outcomes by focusing on equitable access and treatment by implementing focused strategies that lessen their impact and advance equity in care delivery. The overall goal of reducing readmissions by 10% has been identified as one of the main strategies for better patient outcomes. Patients are assessed for risk of readmissions and followed by case managers when moderate to high risk is identified. The Community Memorial Case Management department is collaborating with a transitional care management program for Medicare patients discharged to home, which involves initial contact within 48 hours, and 30-day follow up for medication reconciliation and support. Patients also receive post-discharge care through our Transition Care Management program at our Community Memorial Health Centers.

Community Memorial Hospital – Ventura and Community Memorial Hospital – Ojai both received Geriatric Emergency Department Accreditation by ACEP in 2025. This accreditation is awarded to organizations who demonstrate their commitment to adopting best practices for older adults in the emergency department and provide the best care possible for the community's older adults.
