The CalRHT program is supported by the Centers for Medicare & Medicaid Services (CMS) of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $233,639,308.46 with 100 percent funded by CMS/HHS. The contents are those of CalRHT and do not necessarily represent the official views of, nor an endorsement, by CMS/HHS, or the U.S. Government.
CalRHT Grant Funding Opportunities
The California Rural Health Transformation (CalRHT) program is composed of three initiatives, each with its own set of programs. Transformative Care Model
The Accelerator Partner Program is proposed as support for hospitals or other organizations in rural regions that demonstrate strong readiness and commitment to implement the CalRHT Transformative Care Model (TCM) through regional care collaboratives and hub-and-spoke relationships which will seek to strengthen primary and maternity care in rural parts of the state.
Accelerator Partners will strengthen TCM performance metrics in their service areas while serving as testing sites for innovative care models and evidence-based approaches. Accelerator Partners will demonstrate how integrated service delivery can strengthen access, quality, and sustainability in rural and Remote (FAR) communities.
Lessons learned from these Accelerator Partners will inform statewide scaling and continuous improvement of the regional care collaboratives and hub-and-spoke framework in future years.

Workforce Development
The Workforce Development Recruitment, Retention, and Relocation Program (WDRRR) aims to strengthen and sustain the rural healthcare workforce by improving recruitment, retention, and training for health professionals, with a focus on those who provide primary care and maternal health services. WDRRR will fund eligible organizations to retain health professionals already practicing in rural communities and to recruit or relocate eligible health professionals to qualifying rural sites as defined by the Health Resources and Services Administration (HRSA). HCAI is preparing for the WDRRR Request for Application (RFA) to launch in July.

Technology & Tools
EHR Modernization Grants fund the foundational systems required for basic data exchange by ensuring rural facilities can purchase and sustain modern, secure EHRs, and funding is directed to the entities and activities that support implementation and scale. Allowable uses are limited to Core EHR Modernization, including adoption and configuration of a first-time EHR or replacing an existing EHR.
